The Bridge Clinic Advanced Practice Provider will play a Critical role in this innovative care team focused on addressing health disparities in our community. Working closely with the Bridge Clinic Physician Lead and community partners, this provider will support initiatives to improve access and provide comprehensive care to marginalized individuals who have complex health needs with a special focus on individuals experiencing homelessness. Care will be provided both in a clinic setting collocated with multiple specialists, as well as a mobile setting alongside our community partners. A successful candidate will have a strong commitment to health equity, an interest in building partnerships within the community, and compassion and sensitivity in working with marginalized communities. Additionally, a desire to collaborate closely with case managers, community health workers, other health care professionals and community partners to coordinate care will be paramount to the patient-centered, whole person care for the individuals we care for.
The Bridge Clinic Advanced Practice Provider will work under the Bridge Clinic Physician Lead and provide direct care within the bridge team in a clinic setting, mobile setting and respite care setting. This position aligns with Central Health's deep commitment to advancing health equity and improving health disparities in our community. This position is considered on-site.
ResponsibilitiesEssential FunctionsProvide direct patient care to patients in a clinic setting, mobile setting or respite setting.Collaborate with the community partners and their case managers, CHWs, etc to provide care and support of individuals in the bridge program.Collaborate with attending physician to provide care with bridge team.Participate and lead quality and care review meetings for patients in the bridge program.Communicate effective information to patients, families, colleagues, nursing and other health care professionals, as appropriate.Prepare and provide necessary timely and accurate reports and forms, as may be required by Central Health or facility in the performance of medical services.Perform medical and administrative services under general guidance and minimal supervision with accountability for specific organizational-level goals.Work closely with families of diverse patient populations.Help develop and execute the planning, implementation and evaluation of service delivery, patient experience at our mobile sites.Prioritizes duties and responsibilities, demonstrating strong organization and time management skills.Demonstrates excellent verbal and written communication skills, assuring appropriate confidentiality is always maintained.Interacts with others in a positive, professional manner, contributing to a positive team environment.Performs other duties as assigned.
QualificationsMINIMUM EDUCATION: Graduation from an accredited school of physician assistants OR master’s degree from an accredited school of nursing.PREFERRED EDUCATION: REQUIRED CERTIFICATIONS/LICENSURE: Holds and maintains these certifications as a professional. Lapsing/expiration of these certifications/licensure will result in suspension of work:• Unrestricted license to practice as a Physician Assistant in the State of Texas OR unrestricted license to practice nursing in the State of Texas/current credentialing as an Advanced Nurse Practitioner by the Texas Board of Nurse Examiners.• Current Drug Enforcement Agency (DEA) for the purpose of writing prescriptions.• Current Healthcare Provider Cardiopulmonary Resuscitation (CPR) through American Heart Association.• Current Basic Life Support Certification for Healthcare Providers through American Heart Association.
REQUIRED COURSES/COMPLETIONS (e.g., CPR): Basic Life Support (BLS) ‐ Obtained through approved American Heart Association or Red Cross.
