Wound Care Director

Position Summary

Aliya Healthcare is seeking an experienced and compassionate Wound Care Director to oversee the assessment, prevention, treatment, and ongoing management of wounds across our skilled nursing facilities. The Wound Care Director will work closely with nursing leadership, physicians, nurse practitioners, therapists, dietitians, and facility staff to ensure high-quality, evidence-based wound care and positive resident outcomes.

The ideal candidate will have strong clinical expertise in wound management, experience in a skilled nursing or long-term care setting, and the ability to educate and lead clinical teams.

Key Responsibilities
  • Oversee and coordinate wound care programs across assigned facilities.
  • Conduct comprehensive wound assessments and develop individualized treatment plans.
  • Monitor wound healing progress and revise interventions as clinically appropriate.
  • Collaborate with physicians, advanced practice providers, nursing staff, dietitians, and other members of the interdisciplinary team.
  • Ensure accurate and timely wound documentation in accordance with facility and regulatory requirements.
  • Monitor wound-related clinical outcomes and identify opportunities for improvement.
  • Provide education and hands-on training to nursing staff regarding wound prevention, assessment, treatment, and documentation.
  • Assist facilities with developing and maintaining effective wound prevention protocols.
  • Review wound documentation, treatment orders, measurements, photographs, and progress notes for accuracy and compliance.
  • Participate in Quality Assurance and Performance Improvement (QAPI) initiatives related to wound care.
  • Help identify residents at increased risk for pressure injuries and develop appropriate prevention strategies.
  • Ensure appropriate use of wound care products, supplies, and treatment modalities.
  • Maintain knowledge of current wound care standards, evidence-based practices, and regulatory requirements.
  • Participate in clinical meetings, audits, and facility assessments as needed.
  • Support nursing leadership with wound-related surveys, audits, and regulatory compliance.
  • Maintain confidentiality and comply with HIPAA and all applicable federal and state regulations.

Qualifications
  • Current Registered Nurse (RN) license in the State of Illinois.
  • Bachelor's degree in Nursing preferred.
  • Minimum 3 years of clinical nursing experience, preferably in skilled nursing, long-term care, or post-acute care.
  • Previous wound care experience required.
  • Experience overseeing or coordinating wound care programs strongly preferred.
  • Wound care certification such as WCC, CWCN, CWS, or CWOCN preferred.
  • Strong knowledge of pressure injuries, diabetic wounds, venous and arterial ulcers, skin integrity, and wound prevention.
  • Knowledge of CMS, IDPH, and other applicable skilled nursing regulations.
  • Experience with electronic medical records and clinical documentation systems.
  • Strong leadership, communication, organizational, and critical-thinking skills.
  • Ability to travel between assigned Aliya Healthcare facilities as needed.

  • Skilled Nursing Facility (SNF) experience
  • Long-term care or post-acute care experience
  • Wound Care Certified Nurse (WCC)
  • Certified Wound Care Nurse (CWCN)
  • Certified Wound Specialist (CWS)
  • Certified Wound, Ostomy, Continence Nurse (CWOCN)
  • Experience with QAPI and quality improvement initiatives
  • Staff education and clinical leadership experience
