Qualifications

• Active BLS certification from the American Heart Association
• Current professional nursing license in the applicable state
• Preferred specialty-related certifications for specific roles
• Required experience in home health care
• Some positions may require prior experience in an acute care environment
• Minimum of 2 years nursing experience

Responsibilities

• Manage a diverse caseload of patients requiring daily in-home care
• Provide individualized attention and assessment to each patient
• Coordinate patient care plans in collaboration with interdisciplinary teams
• Demonstrate effective time management and task prioritization skills
• Educate patients and families about care procedures and health management
• Document patient progress and updates thoroughly
• Advocate for patients’ needs and preferences in care settings
