Advanced Practice Provider (NP/PA) - Transitions Care | UNC Wayne - Goldsboro, NC

Job Summary: UNC Health is seeking an experienced Advanced Practice Provider (APP) to support care transitions in the Wayne region. This role will focus on post-discharge patient care, ensuring timely follow-up and continuity across care settings to improve outcomes and reduce readmissions. In addition to managing transitions-of-care visits, the APP will provide coverage for overflow acute care appointments as needed. This position will be based in the Goldsboro area.

Responsibilities:

  • Provide comprehensive post-discharge follow-up care for patients transitioning from hospital to outpatient settings
  • Perform patient assessments, including history, physical examination, and medication reconciliation Develop and implement individualized care plans in collaboration with physicians and care teams
  • Coordinate care with primary care providers, specialists, and community resources Educate patients and families on treatment plans, medications, and self-care strategies
  • Manage overflow same-day or acute care visits as needed
  • Support initiatives to improve care transitions, patient outcomes, and reduce hospital read missions
Qualifications:
