Title: System Pharmacist - Clinical: Internal Medicine/Transition of Care

Summary Details:
This is a patient care-focused role with limited administrative duties, responsible for providing pharmacotherapy recommendations aligned with Atlantic Health System's strategic initiatives. This is a hybrid position, based on-site at the corporate office in Morristown with opportunities for remote patient care activities. The role supports multiple sites, providing pharmacist-specific pharmacotherapy services across the system. The pharmacist provides pharmaceutical care to patients across Atlantic Health inpatient settings, ensuring safe, effective, and evidence-based medication therapy.

Key Responsibilities
  • Review and verify medication orders for appropriateness, safety, and accuracy
  • Monitor therapeutic outcomes and provide clinical recommendations to the healthcare team
  • Collaborate with healthcare professionals across all sites to optimize patient care
  • Participate in quality improvement and patient safety initiatives
  • Ensure compliance with hospital, departmental, and regulatory policies

Job Responsibilities / Essential Functions
  • Reviewing patient medication regimens and providing recommendations to optimize therapy based on clinical guidelines and best practices
  • Conducting medication reconciliation to ensure accuracy and safety during transitions of care.
  • Collaborating with healthcare providers to develop and implement patient-specific care plans.
  • Monitoring patient outcomes and adjusting therapy as necessary to achieve therapeutic goals.
  • Contributing to quality improvement initiatives related to medication use and patient safety.
  • Documenting interventions, clinical assessments, and recommendations in the electronic health record.
  • Staying current with developments in pharmacotherapy and evidence-based medicine.


Qualifications

Required qualifications:
  • Graduated from accredited School of Pharmacy with PharmD
  • PGY1 pharmacy residency training OR 2 years of clinical pharmacy experience in the assigned area
  • Current active registration and licensure with the State of New Jersey Board of Pharmacy
