Additional Qualifications:
- Master of Social work required.
- LCSW licensure required within 6 months of job entry date.
- Social Work Supervisors hired prior to 2001 may remain in their current role without licensure, however they must directly report to an LCSW.
- Qualified Supervisor status to be able to supervise MSW interns for LCSW licensure preferred.
- Demonstrates expert knowledge in the application of social work family centered approach principles to hospitalized patients with complex transition of care needs.
- Knowledge of crisis intervention, interdisciplinary collaboration, and case management principles, medical terminology, procedures and community resources.
- Ability to communicate effectively, maintain objectivity, deal sensitively with a diverse patient population and manage staff.
- Previous experience in discharge planning, knowledge of clinical and social factors that affect the patient's functional status at discharge, socioeconomic needs, patient and family centered care approach.
Minimum Required Experience:
- Master of Social work required.
- LCSW licensure required within 6 months of job entry date
- Qualified Supervisor status to be able to supervise MSW interns for LCSW licensure
- 3 years of experience as a Social Work Case Manager required, preferably in an acute care setting
