Job DescriptionProvides leadership and supervision of the day-to-day clinical and administrative operations of the care management team. Supervises Care Management (CM) staff to include nursing, social work, patient care coordinators and other non-clinical staff to achieve the goals for the organization; collaborates directly with inpatient and ambulatory clinicians and care teams, as well SMG and PCN leaders to support cost-effective high-quality medical services.
Required qualifications:- Washington Registered Nurse License upon hire
- 5 years clinical nursing, including acute care hospital and/or ambulatory clinic experience.
Preferred qualifications:- Bachelor's Degree
- 3 years Case Management experience including care management, care coordination, discharge planning, or population health.
- 2 years progressive supervisory experience.

These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
