Responsibilities

  • Perform Start of Care (SOC) visits and initial assessments.

  • Administer and monitor IV/infusion therapies as ordered.
  • Complete accurate and timely clinical OASIS documentation.
  • Coordinate care with physicians and interdisciplinary team as a Case Manager.
  • Educate and empower patients and caregivers.

Full Job Description
*This is a full-time position that will service the East Valley area (including Tempe, Mesa, Chandler, Gilbert, and Queen Creek). *Annual salary range $85000 - $90000+ per year (depending on experience), plus travel expenses. Your Impact As a Home Health RN, you’ll be the heart of patient care, bringing clinical expertise and compassion directly into the home. This role combines Start of Care (SOC), wound care, and infusion therapy responsibilities, providing high-quality nursing care to patients in their homes.
