Job Description

To perform the following tasks and clinical functions in an efficient and cost-effective manner as assigned in order to help patients/families/significant others respond to their treatment needs; psychosocial screening and assessment, discharge planning, linkage to community resources, crisis intervention, patient/family counseling and psycho-educational philosophy, standards, objectives and policies of the Department and the Hospital Center.

Responsibilities

1. Help patients/families to meet concrete needs such as obtaining services, housing or employment

2. May assist in obtaining medical equipment and support services

3. Gather information about community resources and services and makes appropriate referrals with liaison and follow-up

4. Assists with resumption of home care services

5. Perform specific assignments relevant to particular programs

6. Perform community based tasks to facilitate discharges

7. Document in the medical record in accordance with standards set for the Department of Social Work and/or regulatory agencies

8. Identify high risk situations such as child abuse, elder abuse, domestic violence and informs supervisor of these cases

9. Provide patient/family education and encourages participation in care plan

10. Maintain knowledge of social service policies related to benefits and entitlements and able to assist patients to secure needed services

Qualifications

Related experience in the social service and/or health care field, preferably one year in a medical setting.

The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $36.24 - $41.38 Hourly. Actual salaries depend on a variety of factors, including experience, education, and operational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
