As our RN Patient Care Coordinator in Wheaton, IL, you'll keep home health care moving smoothly-connecting the dots so patients get timely, compliant, high-quality services. You'll be the clinical point person who keeps communication crisp and care plans on track.
Key responsibilities include serving as the primary clinical liaison for field clinicians, physicians, referral sources, and patients/families; coordinating and communicating care needs, schedule updates, and follow-ups; supporting clinicians with clinical questions, orders clarification, and home health protocols; initiating and coordinating care plans aligned to physician orders and Medicare requirements; monitoring patient status and escalating changes to the interdisciplinary team; ensuring timely OASIS, visit notes, and documentation submission; and assisting with case conferences, reassessments, and care plan updates.
Qualifications:
- Active Illinois RN license
- 1-2 years nursing experience (home health preferred)
- Working knowledge of Medicare home health regs, OASIS, and care coordination
- EMR proficiency (WellSky/Kinnser)
