Nurse Coordinator, Home Parenteral and Enteral Nutrition

The Nurse Coordinator supports the UNC Home Parenteral Nutrition (HPN) and Enteral Nutrition (EN) programs by coordinating care for patients receiving nutrition support therapy. This role is responsible for outpatient triage, patient education, and support of inpatient transition to home TPN/EN, with a strong focus on patient safety, CLABSI prevention, and optimization of nutrition therapy.

The ideal candidate will have experience in triage nursing, central line care, and patient education, and will be able to assess both the physical and psychosocial readiness of patients for home TPN/EN. The Nurse Coordinator works closely with patients, pharmacists, advanced practice providers, surgery, gastroenterology, registered dietitians, TPN contractors, and other members of the interdisciplinary team to ensure timely, evidence-based care.

Responsibilities:

Responsibilities include managing the HPN/EN access triage phone line, responding to Epic messages and chats, reviewing labs and order requests, placing cosign orders, and managing the Home PN Outlook inbox.

This is a hybrid position based primarily at UNC Main Hospital. The triage line is covered Monday through Friday, 8:00 a.m. to 5:00 p.m., with no weekend or night shifts. The Nurse Coordinator will work alongside the existing nurse to support the inpatient and outpatient UNC HPN/EN program.
