Summary:
Provides chronic condition management and health coaching to members with disease risk indicators to achieve health risk reduction, improved quality outcomes, enhanced member experience, and cost-efficiency.

Formal Education Required:
a. Bachelor's degree in nursing or equivalent education and experience.
b. Registered Nurse (RN) state license in good standing.

Experience & Training Required:
a. A minimum of three (3) years experience to include care coordination, chronic condition management, acute or managed care.

3. Other Skills, Competencies and Qualifications:
a. Ability to perform tasks with little supervision
b. Collect and analyze data from diverse sources; making recommendations and/or conclusions based on analyses
c. Computer literacy to locate information from multiple sources, documentation, remote work, etc.
d. Ability to effectively interact with populations of patients/customers with an understanding of their needs for self-respect and dignity

4. Level of Physical Demands:
a. Light: Exerts up to 20 pounds of force occasionally and/or up to ten pounds of force frequently, and/or a negligible amount of force continuously.
b. Sit for prolonged periods of time.
c. Bend, stoop, and stretch.
