This role supports MultiCare's Bed Readiness Program through system-wide referral coordination, placement, and active case management of patients in the INW and Yakima markets. The position requires an in-person Case Manager serving the Yakima or Inland Northwest geographic areas.
FTE: 1.0, Shift: Day, Schedule: Monday - Friday
Position Summary
The Hospital Case Manager RN is responsible for high quality cross continuum care coordination. This individual works with physicians/APP, peers, community colleagues, and others to orchestrate care across the health care continuum, to identify opportunities to continually improve patient care and services, to improve population health, and to achieve collaborative practices that exemplify MultiCare Health System's commitment to patient centered care and community engagement.
Responsibilities
347You will ensure that the patient and family experiences medically appropriate transitions of care, and an integral and essential component of the care delivery process347
347You will advocate for patients and families, including transition to lower level of care when inpatient care is no longer needed347
347You will partner with and supports physicians/APP in patient care activities and care transitions347
347You will utilize critical thinking in all interactions347
347You will be accountable for transitions of care and care across the continuum347
Requirements
347Bachelor's degree in Nursing, or obtained within five (5) years from the initial date of hire; RN with 15+ years of service or internal candidates hired before January 1, 2021 may be considered in satisfaction of this requirement in lieu of an education agreement347
347Advanced degree in Nursing (MSN, PhD, DNP, ARNP) preferred347
347Current Registered Nurse license in Washington State or Multistate License endorsement (MLS)347
347Four (4) years recent hospital case management experience347
347Five (5) years of recent clinical hospital/ambulatory experience347
347Healthcare financial and reimbursement knowledge preferred347
347Leadership and emergency services experience a plus347
347Consideration may be given to internal candidates not meeting the minimum qualifications347

Pay and Benefit Expectations
We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $101,407.00 - $177,821.28 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant's years of experience align.
