Job Function: - Day Shift.
- Manage an OPWDD caseload of 18-25 individuals, ages 20 through adulthood, with moderate to profound intellectual and developmental disabilities (IQ 40-70).
- Travel between Red Hook and Tivoli/Columbia County residential group homes to conduct assessments, monitor health needs, and support staff.
- Maintain accurate health records and ensure compliance with developmental disabilities nursing standards.
- Oversee and support medication administration through AMAP-certified staff.
- Attend medical appointments as needed and collaborate with healthcare providers.
- Participate in interdisciplinary team planning and care coordination.
Job Requirements:Job Requirements: - Active Registered Nurse (RN) license in the state of practice.
- Associate or Bachelor's degree in Nursing (BSN preferred).
- Prior experience in case management, utilization review, or care coordination preferred.
- Proficiency with electronic medical records (EMR) systems.
