Job Summary

We are seeking an experienced Registered Nurse Case Manager for a 13-week travel assignment in Juneau, Alaska.

This role is ideal for an RN with recent hospital case management experience who is skilled in care coordination, discharge planning, utilization review, and interdisciplinary collaboration.

Candidates must have an active Alaska RN license and recent case management experience within the past 2 years.

Key Responsibilities
• Coordinate patient care throughout the hospital stay and transition to the appropriate next level of care.
• Develop and manage individualized discharge plans based on patient needs.
• Collaborate with physicians, nurses, social workers, rehabilitation teams, and other healthcare professionals.
• Assess clinical, psychosocial, financial, and discharge planning needs.
• Identify barriers to discharge and work proactively to resolve them.
• Coordinate post-acute services such as skilled nursing, home health, rehabilitation, durable medical equipment, and outpatient follow-up.
• Communicate with patients and families regarding care plans and discharge expectations.
• Support safe and timely transitions of care.
• Participate in interdisciplinary rounds and care conferences.
• Review patient progress and update care plans as needed.
• Assist with utilization management and medical necessity review as appropriate.
• Coordinate with insurance providers, payers, and external agencies when required.
• Maintain accurate and timely clinical documentation.
• Support compliance with hospital policies, regulatory requirements, and case management standards.
• Advocate for patient needs while promoting appropriate use of healthcare resources.

Required Qualifications

Candidates must have:
Active and unrestricted Alaska Registered Nurse (RN) license
• Associate Degree in Nursing from an accredited program
• Current BLS certification
• Recent RN Case Management experience within the past 2 years
• Experience working in an acute-care hospital setting
• Strong discharge planning and care coordination skills
• Excellent communication and organizational skills
• Ability to manage multiple patients and competing priorities
• Strong interdisciplinary collaboration skills

Preferred Qualifications

Strong candidates may also have:
• Bachelor of Science in Nursing (BSN)
• Previous travel nursing experience
• Utilization review experience
• Experience with complex discharge planning
• Experience coordinating post-acute services
• Knowledge of Medicare, Medicaid, and commercial insurance processes
• Familiarity with case management software and electronic health records
• Certification in Case Management, such as CCM or ACM
