Job Summary

The Community Health Nurse is responsible for providing skilled professional nursing care to the patients assigned to him/her in a comprehensive manner promoting quality, continuity and safety.

Core Job Responsibilities

  • Provides professional nursing care within the team concept utilizing all elements of the nursing process to ensure a high quality of professional service and adhering to VNA standards and policies.
  • Strives for a safe environment for the patient; adheres to standard precautions.
  • Initiates and carries out continuous assessment of physical, emotional, social, cultural, economic and environmental needs of the patient and their families.
  • Observes the patient for changes in condition and reports to the nursing supervisor and the physician.
  • Plans and provides for comprehensive home health care services to patient through formulation and revision of the nursing care plan, making appropriate service referrals within and outside the agency.
  • Plans for patient discharge from services.
  • Supervises supportive care staff and delegates responsibilities appropriately.
  • Promotes good health practices helping the patient in accepting and assuming responsibility for attaining the highest level of independent functioning possible.
  • Implements orders for medications and treatments as prescribed by the physician, submitting and maintaining appropriate medical orders.
  • Instructs patient and the family in self-care techniques, diet, medications, and other aspects of care as outlined in the physician's orders.
  • Completes, maintains and submits accurate, legible and relevant nursing paperwork regarding the patient's condition and all care and treatment given.
  • Monitors patient expectations and satisfaction.
  • Accepts assignments and performs duties within the limits of preparation and experience, seeking and obtaining sufficient information necessary to function effectively in the home health care setting.
  • Participates in appropriate agency meetings and the quality improvement and utilization review activities of the organization.
  • Assumes responsibility to maintain and upgrade professional knowledge and skills through attendance and participation in continuing education, in-service programs, professional conferences and community and agency.
  • Supports the Agency's Compliance Program, ensuring that all information is kept confidential and is respectful of the Patient's Bill of Rights.
  • Strives for a safe environment by adhering to the Agency Safety Program.
  • Responsible for compliance with HIPAA regulations, to include attendance at regular and updated HIPAA and privacy training.
  • Perform other duties as required.


Education/Experience Requirements

REQUIRED:
  • BSN from an accredited -nursing program.
  • At least one year of experience working as a registered nurse in the acute care setting preferably just prior to application.


Licensure/Certification Requirements

REQUIRED:
  • License and current registration to practice as a registered professional nurse in New York State.
