Our home is looking for an open minded, motivated healthcare professional who seeks to improve the lives of our residents by providing high quality assessments, care and supervision. This position is in assisted living facility in Troy, NY where an understanding of and passion for the elderly and/or mental health population is a must.

Job Responsibilities Include but are not limited to:
• Performing admission assessments and reassessments for all residents in the home
• Developing, monitoring, and reevaluating plans of care
• Day to Day oversight of personal care aides
• Interacting with primary care, specialty and hospital providers to ensure resident needs are being met
• Following all regulatory guidelines
• Triaging and assessing residents to determine healthcare needs
• Participating in continuous quality improvement program, quality assurance activities and regular chart audits

Job Qualifications:
• Active NYS Registered Nurse License
• 1-3 years experience in an Assisted Living or Long Term care setting, high preferred but not required
• Experience with developing plans of care preferred
• Experience with various assessments such as UAS, OASIS, PRI & Screen preferred

$80,000 - $100,000 Annually
