Lead Registered Nurse

Assisted Living Program (ALP) / LHCSA
Position Overview

The Lead RN is responsible for overseeing clinical operations, resident assessments, regulatory compliance, and nursing coordination within the Assisted Living Program (ALP) and Licensed Home Care Services Agency (LHCSA). This role ensures high-quality resident care, accurate documentation, staff oversight, and compliance with Department of Health (DOH) and organizational standards.
Key Responsibilities
Clinical Oversight & Resident Care
  • Conduct in-person nursing assessments for new admissions, recertifications, significant change evaluations, and post-hospitalization returns
  • Complete Start of Care, UAS assessments, reassessments, and care plan development
  • Address urgent and emergent resident clinical needs and coordinate services as needed
  • Complete fall risk, nutritional assessments, and ongoing chart audits
  • Participate in DOH surveys, audits, and performance improvement initiatives
Documentation & Compliance
  • Maintain accurate and timely documentation in HCM, CompuCare, and state systems
  • Upload and manage required resident documentation, certifications, and plans of care
  • Ensure compliance with ALP/LHCSA regulations, policies, and assessment timelines
  • Conduct quarterly chart audits and annual chart thinning
Staff Oversight & Education
  • Orient aides to residents and individualized Plans of Care
  • Conduct aide supervisions, skills competencies, and annual compliance requirements
  • Provide clinical education and re-education to caregiving staff as needed
  • Review and sign off on duty sheets and care delivery
Coordination & Communication
  • Attend required nursing meetings (daily, weekly, monthly, quarterly as applicable)
  • Submit weekly Lead RN reports to leadership
  • Collaborate with interdisciplinary teams, case managers, providers, and leadership
  • Manage HealtheConnections access, consents, and medical record retrieval
Required Qualifications
  • Active Registered Nurse (RN) license in good standing
  • Experience in assisted living, home care, long-term care, or related clinical settings
  • Strong knowledge of resident assessments, care planning, and regulatory compliance
  • Proficiency with electronic health records and documentation systems
  • Excellent organizational, communication, and leadership skills
Preferred Experience
  • ALP and/or LHCSA experience
  • Familiarity with UAS assessments, HCM, CompuCare, and DOH requirements
  • Experience supervising and educating nursing and aide staff
Work Schedule
  • Full-time
