Registered Nurse (RN) Case Manager
As an RN Case Manager, you will:
Conduct comprehensive patient assessments, including OASIS documentation
Develop, implement, and evaluate individualized care plans
Provide skilled nursing care and treatments in the home setting
Educate patients and caregivers on disease management, medications, and care plans
Coordinate care with physicians, therapists, and other healthcare professionals
Supervise LPNs and support staff as needed
Complete accurate and timely electronic documentation
Ensure compliance with agency policies, Medicare regulations, and clinical standards
Participate in care conferences, team meetings, and quality improvement initiatives (QAPI)
Qualifications
Active Indiana Registered Nurse (RN) license
Associate Degree in Nursing (ASN) or Bachelor of Science in Nursing (BSN)
Previous home health experience preferred
OASIS assessment experience preferred
Experience using electronic health records (EHR) systems preferred
Strong clinical assessment, communication, and organizational skills
Ability to work independently and manage a patient caseload effectively
Valid driver's license and reliable transportation

Job Types: Full-time
Compensation: $50-$110 per visit or Salary for highly experienced candidates.
Work Location: Field-based, providing care in patients' homes throughout the Indianapolis area
Schedule: Flexible scheduling based on patient needs and caseload
