We are seeking a compassionate and skilled
Home Health Registered Nurse (RN) to provide high-quality, patient-centered care in the home setting. In this role, you will manage a caseload of patients with a variety of medical needs, helping them recover from illness or surgery, manage chronic conditions, and maintain their independence in the comfort of their own homes.
The ideal candidate is clinically confident, organized, and passionate about building meaningful relationships with patients while delivering exceptional care.
Responsibilities
- Conduct comprehensive patient assessments and develop individualized plans of care.
- Provide skilled nursing services, including wound care, medication management, IV therapy, injections, catheter care, and disease management.
- Monitor patient progress and modify care plans based on clinical findings.
- Educate patients and family members on disease processes, medications, treatment plans, and preventive care.
- Collaborate with physicians, therapists, social workers, and other members of the interdisciplinary healthcare team.
- Complete accurate and timely electronic documentation in accordance with agency, state, and federal guidelines.
- Identify changes in patient condition and communicate concerns promptly to the appropriate providers.
- Coordinate community resources and additional services as needed.
- Participate in case conferences and quality improvement initiatives.
- Ensure compliance with Medicare, Joint Commission, state, and agency standards.
Qualifications
Required - Active Registered Nurse (RN) license in the applicable state.
- Graduate of an accredited nursing program.
- Minimum of one year of recent nursing experience.
- Current BLS certification.
- Valid driver's license, reliable transportation, and current auto insurance.
