We are seeking an experienced and compassionate
Nurse Practitioner Supervisor to join a collaborative primary care team dedicated to providing exceptional care to adult and geriatric patients. This leadership role combines direct patient care with clinical oversight, allowing you to make a meaningful impact while helping guide care delivery across an interdisciplinary team.
In this position, you'll work closely with physicians and healthcare professionals to coordinate comprehensive care, manage chronic and acute medical conditions, promote preventive health, and ensure patients receive the highest quality of care throughout their healthcare journey.
What You'll Do
- Provide primary care services to adult patients within an assigned practice site.
- Perform annual wellness visits and routine health assessments.
- Evaluate, diagnose, and manage acute and chronic medical conditions.
- Develop individualized treatment plans and educate patients on disease prevention and chronic disease management.
- Conduct post-hospital follow-up visits, including medication reconciliation and coordination of ongoing care.
- Collaborate with physicians, specialists, nurses, and interdisciplinary team members to ensure seamless patient care.
- Supervise and oversee care plan development and implementation for assigned participants.
- Provide timely same-day access for urgent patient needs through in-person or telemedicine visits.
- Maintain accurate, complete electronic medical records and documentation.
- Support quality improvement initiatives designed to enhance patient outcomes.
- Represent the Medical Director when delegated or during their absence.
- Perform additional clinical and leadership responsibilities as needed.
Qualifications
Required - Graduate of an accredited Nurse Practitioner program approved by the New York State Education Department.
- Active New York State Nurse Practitioner license.
- Active New York State Registered Nurse license.
- DEA Certification.
- AANP or ANCC Board Certification.
- Minimum of two years of clinical experience in primary care, home care, care management, or a related setting.
- Minimum of one year of experience caring for adult patients or willingness to complete organizational training.
- Strong communication, organizational, and critical thinking skills.
- Proficiency with electronic medical records and Microsoft Office applications.
- Medicaid and Medicare Provider IDs in good standing.
Preferred - Bilingual skills.
- Leadership or supervisory experience.
- Experience working with interdisciplinary healthcare teams.
