Salary Range: $35.46- $50.05 per hour
How you'll make an impact in this roleProvide direct nursing care to participants in Program of All-Inclusive Care (PACE) in accordance with established policies, procedures and protocols of PACE organizations.
- Provide direct participant care including acute triage, blood draws and wound care. Assists the care team for provision of participant care including after hours/weekend on-call rotation.
- Participate in the development, implementation, and evaluation of the Interdisciplinary Team (IDT) plan of care and update changes as necessary.
- Attend Interdisciplinary Team (IDT) care plan meetings and Participant/Family meetings as applicable. May perform assessments in a variety of settings including PACE Clinic and/or Adult Day Center.
- Develop and provide education to participants, caregivers, and associates as necessary including end of life discussions. May require travel to participant's place of residence, long term care and assisted living facilities.
What minimum requirements you'll needLicensure / Certification / Registration:
Required Credential(s):
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
- Registered Nurse obtained prior to hire date or job transfer date.
Education:
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
