How you'll make an impact in this role- Design holistic care plans that blend medical necessities with the unique personal preferences and values of each patient and their family.
- Evaluate patient journeys continuously by analyzing medical records and real-time assessments to ensure they are always receiving the appropriate level of care.
- Advocate fiercely for patients by identifying system bottlenecks, removing barriers to care, and unlocking critical recovery resources.
- Lead multidisciplinary communication by maintaining a constant feedback loop with supervisors and transition teams to keep care strategies fluid and effective.
- Serve as a clinical documentation expert, guiding teams on accurate record-keeping and robust post-discharge options tailored to each diagnosis.
What minimum requirements you'll needLicensure / Certification / Registration:
- Registered Nurse credentialed from the Maryland Board of Nursing obtained prior to hire date or job
transfer date required. - BLS Provider preferred. American Heart Association or American Red Cross accepted.
- Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
Education:
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of
hire.
What additional preferences we're seeking- Proven Case Management Experience (minimum of 2 years) specifically centered in a hospital setting, acute care, or population health.
