JOB PURPOSE:

Under the direction of the Regional Medical Director, the Regional Clinical Lead - Physician Assistant is responsible for the professional, clinical, organizational, and administrative aspects of providing high-quality medical care to participants across designated Diagnostic and Treatment Centers and other assigned care settings within a defined region. The role provides advanced practice and clinical leadership coverage based on operational and participant-care needs; supports continuity of site operations; and works collaboratively with physicians, advanced practice providers, interdisciplinary teams, participants, and families to ensure coordinated, person-centered care.

JOB RESPONSIBILITIES:

• Provide Physician Assistant and clinical leadership coverage across designated Diagnostic and Treatment Centers and other assigned care settings within the region, supporting a diverse adult and geriatric participant population.

• Travel regularly among assigned sites and adjust the primary work location based on operational, staffing, clinical, and participant-care needs.

• Support continuity of clinical operations across the region, including coverage for vacancies, leaves, scheduling gaps, increased participant volume, start-of-care activity, and other emerging needs.

• Direct the medical care and treatment of participants within the Physician Assistant's scope of practice and provide clinical oversight and support to members of the interdisciplinary care team.

• Support the needs of site participants, including primary-care and community-based membership, and promote consistent standards of care across covered locations.

• Carry an assigned caseload as a primary care provider and participate in direct medical care in the Diagnostic and Treatment Center, participant residences, and other approved settings.

• Demonstrate clinical presence during start-of-care events and complete or support assessments and follow-up activities as assigned.

• Assess and manage participants with acute changes in condition in a timely manner through face-to-face or telemedicine visits, as appropriate.

• Conduct post-hospitalization and post-emergency-room follow-up visits within required timeframes, including medication reconciliation, treatment-plan development, and coordination with the primary care provider, specialists, and Interdisciplinary Team.

• Identify opportunities to improve the quality, cost, access, and utilization of care for participants across assigned sites and assist with implementing improvement actions.

• Participate in multidisciplinary activities, including case conferences, hospitalization reviews, care-planning meetings, family meetings, and Interdisciplinary Team meetings.

• Develop and participate in utilization review of medical services provided by the Interdisciplinary Team and Diagnostic and Treatment Center.

• Support the development, coordination, and oversight of ambulatory medical services across covered sites.

• Serve as a liaison with hospitals, nursing homes, specialists, community physicians, and other medical institutions to support semiannual care planning, referrals, transitions, and continuity of care.

• Collaborate with Regional Medical Directors, site leadership, physicians, advanced practice providers, and interdisciplinary teams to promote consistent clinical practices and effective handoffs across assigned locations.

• Identify site-level clinical or operational concerns, communicate them promptly to the appropriate leader, and assist with corrective or improvement actions.

• Serve as a clinical resource for staff practicing in the Diagnostic and Treatment Center, including Nurse Practitioners, Physician Assistants, Registered Nurses, and Medical Assistants.

• Facilitate medical education, in-service training, staff development, and clinical teaching for staff, trainees, and students as opportunities arise.

• Participate in the development and implementation of preventive health screening, immunization, quality-assurance, and professional community-outreach initiatives.

• Ensure clinical documentation is complete, accurate, coded appropriately, and entered in the electronic health record within required timeframes.

• Share relevant clinical and operational information with the receiving site or provider to support safe transitions and continuity of care.

• Maintain the confidentiality and security of protected health information and comply with all HIPAA requirements.

• Practice only within the individual's authorized scope of practice.

• Meet the standardized competencies established by the PACE organization for this position before working independently.

• Serve as an active member of the Interdisciplinary Team.

• Perform other duties as assigned.

Schedule: Monday - Friday | 8:00 AM - 5:00 PM

Location: Assigned Region: Manhattan/Bronx or o Manhattan/Brooklyn. Multi-site regional role, the assigned worksite may vary based on operational and clinical needs.

Travel: Regular travel among designated locations is an essential function of the position

QUALIFICATIONS:

Education:

• Graduation from an accredited Physician Assistant program required.

• Formal education, training, or equivalent experience in geriatrics or palliative care preferred.

Experience:

• Residency in Internal Medicine or Family Practice is required.

• Board certification in Internal Medicine or Family Practice.

• No malpractice or loss of license at the DTC.

• Minimum of one (1) year of experience working with a frail or elderly population. An individual with less than one (1) year of experience who meets all other requirements must receive appropriate training from the PACE organization upon hire.

• Experience providing clinical leadership, coordinating care across multiple locations, or supporting interdisciplinary teams preferred.

• Excellent verbal, written, interpersonal, and clinical communication skills.

• Strong clinical judgment, analytical, organizational, and problem-solving skills.

• Ability to manage competing priorities and meet clinical, documentation, and operational deadlines across multiple locations.

• Basic computer proficiency, including Microsoft Office and electronic health record systems.

License and Certification:

• Current, active, and unrestricted New York State Physician Assistant license required.

• Current DEA registration required.

• Current NCCPA board certification required.

• Current Basic Life Support certification required.

Additional Requirements:

• Be legally authorized and appropriately licensed, registered, or certified to perform the required functions in New York State.

• Be medically cleared for communicable diseases and have all required immunizations up to date before engaging in direct participant contact.

• Maintain Medicaid and Medicare Provider IDs in good standing.

• Ability and willingness to travel regularly among assigned locations within the designated region and to report to different sites based on operational need.

Language: Bilingual skills preferred, based on the needs of the assigned region.

Physical Requirement:

Individuals must be able to sustain the physical requirements essential to the job. These include, but are not limited to:

• Standing - Up to six (6) hours per day.

• Sitting/Stationary Positions - Up to six to eight (6-8) hours per day for consecutive periods.

• Lifting/Push/Pull - Up to 50 pounds of equipment, baggage, or supplies and the ability to assist or lift participants safely in accordance with OSHA guidelines.

• Bending/Squatting - Ability to bend or squat safely when caring for participants or using medical supplies and equipment.

• Stairs/Steps/Walking/Climbing - Ability to maneuver stairs, climb up or down, and walk to access work areas, including travel and walking among sites and other work locations throughout the day.

• Agility/Fine Motor Skills - Ability to operate equipment, devices, instruments, and tools, including typing and handling medical supplies and equipment.

• Sight/Visual Requirements - Ability to visually assess participants, read orders, and type or write documentation accurately.

• Audio, Hearing, and Language Motor Skills - Ability to listen attentively, accurately process and document information, and communicate clearly with participants, community members, providers, and colleagues.

• Cognitive Ability - Ability to demonstrate sound decision-making, judgment, reasoning, cognitive processing, and analysis in performing the essential functions of the position.
