Overview

Ready To Drive he Campaigns That Connect Health Plans to the Members Who Need Them Most?

Why This Role Matters

Activate is Personify Health's intelligent outreach solution — a data-driven platform that helps health plans close care gaps, drive medication adherence, and improve Star Ratings by reaching the right members at the right time. Getting it implemented well is everything. As a Project Manager for Activate, you'll own the end-to-end delivery of campaign implementations for health plan clients, ensuring that every configuration, data specification, and workflow is built to perform from day one.

This role sits at the intersection of project discipline and product expertise. You'll need to understand Activate's core logic and outreach capabilities well enough to consult, configure, and problem-solve — while keeping clients, internal teams, and timelines moving in lockstep.

What You'll Actually Do

Lead end-to-end Activate campaign implementations: Own project plans, milestone tracking, and cross-functional coordination for health plan clients from kickoff through go-live — managing scope, timelines, budgets, and stakeholder alignment throughout.

Configure and document solution designs: Perform application configuration per implementation plan, document core logic and data specifications, and design workflows that translate client goals into campaign execution across Activate's multi-channel outreach capabilities.

Own scope and budget integrity: Maintain each project's statement of work, manage change requests, estimate timeline and cost impacts, and track Professional Services hours to keep engagements on target.

Keep clients and stakeholders aligned: Lead client-facing meetings, present project updates, and serve as the day-to-day point of contact for health plan clients and brokers — navigating questions, concerns, and escalations with clarity and confidence.

Protect quality and enable clean handoffs: Deliver defect-free outputs at every stage, document SOPs for ongoing operations, and transition completed implementations to Production Support without gaps.
