Full Job Description
The Program Director is responsible for the overall program operation of Adult Home Referral program which includes management & supervision of staff, overseeing the development and implementation of program enhancements, monitoring and compliance, evaluation of policies and implementing QI and incident report procedures for the program. The Program is characterized by a team approach, supervision of clinical staff, time unlimited services, crisis availability, and referral procedures. The Program Director is on call 24/7. This position is eligible to participate in Comunilife's generous benefit package including 4 weeks of vacation within the first year of employment. The salary range for this position is between $80,000 a year and $90,000 a year.

Essential Functions:

Program Management:
  • Review and accept new and renewal leases or assignment of leases from other contractors
  • Plans and administer daily operation of the program
  • Provide oversight to ensure that a minimum program monthly occupancy rate of 95% is maintained
  • Regularly review and monitor assessors output activities to ensure that benchmark are met.
  • Regularly review Inreach work performance in meeting the monthly contractual outcome.

Staff Management:
  • Responsible for staff program assignments and supervision of program activities to ensure that clients receive services in accordance with their needs
  • Assist and participate in the recruitment and retention of staff
  • Monitor Inreach list, generated in CAIRS to ensure that all efforts to engage class members have been properly documented. Provide and maintain records of staff trainings on an annual basis to increase their knowledge and understanding of OMH Adult Home Initiative, older population, psychotropic medication and clinical intervention with the older adult population, mental illness and substance use intervention, and harm reduction techniques.

Program Compliance:
  • Responsible for enforcing policies and procedures in compliance with Federal, State and City Laws, rules and regulations
  • Ensure that Quality Improvement Policies and Procedures are follow as required
  • Ensure program compliance with OMH contract requirement
  • Provide oversight in maintaining and securing individual files and records to ensure that files are kept complete, current, and readily identifiable from other Comunilife programs. Ensure that all client records are maintained by Comunilife for a minimum of seven (7) years after the expiration of the contract.
  • Maintain all documents necessary for a quality assurance and improvement review by Comunilife or OMH.
  • Provide reports as requested by Comunilife and/or OMH. Ensure timely completion of post incidence, benchmark and weekly occupancy reports. Provide oversight in conducting an annual Client Satisfaction Survey and provide surveys to OMH if requested
  • Ensure policies, principles, and procedures are in agreement with OMH requirements, guidelines, regulations and directives

Education and Experience Requirement(s)

A Master's Degree in social work, public health, health administration, nursing or related field; and three(3) to five (5) years of administrative and supervisory experience operating Scatter Site Housing Program(s);

or Bachelor's Degree in social work, public health, health administration, or related field; and five(5) to seven (7) years of administrative and supervisory experience operating Scatter Site Housing Program(s).

Five or more years' experience in providing direct services related to the treatment and recovery of older adult population with serious chronic illness including medical, mental health and substance use. Bilingual-Spanish preferred. Gerontology training a plus

Certifications & Licenses Required

LMSW/LCSW, RN, and/or CASAC.
