Compensation Range: $75,000 - $87,500 yearly commensurate with experience

Benefits:
We have three comprehensive healthcare plans to choose from based on your priorities and budget. Housing Works covers most of the plan; you pay a portion, based on your salary. Staff begins accruing PTO immediately for a total of up to 30 days earned in the first year. We offer employees an educational benefit. This money is available for tuition loan reimbursement, tuition costs, and text books.

Overview:

The Program Coordinator (PC) provides programmatic and clinical leadership for the Cylar House supportive congregate housing program. The PC is responsible for ensuring regulatory compliance, supervising case management staff, maintaining high-quality resident-centered services, and supporting housing stability outcomes. The role balances clinical oversight, staff supervision, quality assurance, and operational coordination in alignment with Housing Works' mission, values, and applicable funder requirements.

Responsibilities:

Primary:
  • Provide leadership and oversight of all programmatic and clinical operations to ensure compliance with applicable funder regulations, agency policies, and best practices in supportive housing.

Secondary:
  • Provide documented supervision and professional development to Housing Case Managers and related program staff.

Other functions that may be required with this position include:
  • Provide leadership in crisis intervention and direct resident support as needed, especially around promoting housing stability and safety.
  • Represent the program at interdisciplinary meetings, treatment planning sessions, and related events.
  • Ensure timely completion of resident assessments and service plans within 30 days of admission, with updates completed in accordance with funder requirements.
  • Facilitate monthly community meetings and support an active Resident Advisory Board.
  • Complete required internal activity forms and maintain accurate resident records for admissions, income updates, and discharges.
  • Support recruitment of staff, including interviewing, making hiring recommendations, and identifying training needs to implement capacity building initiatives.
  • Provide structure and accountability in alignment with agency policies and procedures, including the Residential Code of Conduct.
  • Conduct weekly documented supervision and provide ongoing support to applicable program staff.
  • Review and approve resident assessments, service plans, case conference documentation, and progress notes to ensure quality, accuracy, resident centered engagement, and compliance.
  • Coach staff in harm reduction, trauma-informed care, professional boundaries, motivational interviewing, and crisis intervention.
  • Review monthly rent rolls and collaborate with residents and staff to address arrears and housing stability concerns.
  • Conduct internal audits of resident charts to ensure accuracy and compliance with applicable funder and agency requirements.
  • Ensure accurate and timely data entry into all required electronic case management and reporting systems, maintaining complete and compliant records for admissions, income changes, service plans, inspections, and discharges in alignment with applicable funder data quality standards.
  • Develop recreational and community-centric activities and events to support the residents in the program.
  • Foster a safe, equitable, and inclusive program environment in partnership with agency leadership
  • Actively participate in advocacy and direct actions aligned with Housing Works' mission to end homelessness and HIV/AIDS.
  • Performs other related duties as assigned.

KPIs:
  • Supervision: provide documented weekly supervision with direct reports.
  • Compliance: 100% of assessments, reassessments, and service plans reviewed and approved within the first 30 days of the residents' occupancy, with updates completed in accordance with funder requirements.
  • Client Rent Responsibilities: Review all monthly rent reports within five (5) business days, complete and document follow-up for 100% of missed rent cases and initiate a multidisciplinary case conference for residents with three (3) consecutive months of unpaid rent.
