  • Collaborate with Reliant Medical Group's medical specialists including behavioral health and dermatology.
  • Utilize a state-of-the-art EPIC EMR System for efficient patient management.

Position Highlights & Primary Responsibilities:
• Patient centered team-based environment
• Will join as a AP Practice Partner working with a patient centered team.
• Team inclues: Physician, AP, RN, MA, and a PSS (admin)
• Provide Care across the continuum, focusing on value-based care and suported by a robust team of specialist
• Behavioral health providers are integrated into the primary care teams
• Fewer patients per day, longer patient visits
• Patient centered team-based environment

Required Qualifications:
• Able to obtain Board Certification as a Physician Assistant or Family Nurse Practitioner (FNP)
• Certification to care for patients of all ages- pediatrics through adults
• Able to obtain Unlimited Licensure in the state of Massachusetts
• A minimum of 2 years of experience working in primary care
• Active DEA License

The salary range for this role is $104,500 - $156,000 annually based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
