Job Description Summary

‎ 

The Nurse Practitioner (NP) End-Stage Renal Disease (ESRD) provides high-quality, patient-centered clinical care for individuals with advanced chronic kidney disease (CKD Stage 4/5) and ESRD across care settings. This role supports CareMore Health's commitment to value-based care, population health, and improved clinical outcomes through comprehensive nephrology-focused assessment, diagnosis, treatment, and care coordination. The NP partners with nephrologists and interdisciplinary care teams to optimize dialysis management, reduce hospitalizations, improve quality of life, and support patients throughout the continuum of kidney disease.

How will you make an impact & Requirements

‎ 

  • Patient Assessment:Conducting physical examinations, taking detailed medical histories, and evaluating patient health status.
  • Diagnosis & Treatment:Diagnosing, treating, and managing acute illnesses (e.g., infections, minor injuries) and chronic conditions (e.g., diabetes, hypertension).
  • Diagnostic Testing:Ordering, performing, supervising, and interpreting laboratory tests and imaging studies (X-rays, EKGs).
  • Prescribing & Procedures:Prescribing medications and non-pharmacologic treatments, as well as performing minor procedures.
  • Health Promotion:Counseling patients on health maintenance, disease prevention, and nutrition.
  • Documentation:Recording accurate patient information in Electronic Health Records (EHR) in compliance with regulations.
  • Collaboration:Working within interdisciplinary teams, consulting with physicians, and providing referrals to specialists as needed
