Position Title: Physical Therapist - Kedzie
Full Time
Day Shift - 8am-4:30pm
Compensation: $80,080K - $112K

The estimated range is the budgeted amount for this position. Final offers are based on various factors, including skill set, experience, location, qualifications and comparisons to colleagues in similar roles.

Position Summary

Evaluates and treats patients using the principles and practices of physical therapy in accordance with the patient's age, developmental level and diagnosis.

Responsibilities
  • Evaluates patients, establishes goals and develops individualized treatment plans.
  • Provides direct patient treatment based on current physical therapy practice and revises treatment plan based on the patient's response.
  • Provides education to patients and family regarding patient abilities and need for assistance and continued therapeutic intervention.
  • Consults with physicians on patient's evaluations, progress, and complications with treatment.
  • Maintains medical and departmental records according to department policies and procedures.
  • Participate in patient care conferences, department meetings and department and hospital in-services.
  • Communicate with other professional staff and patient's family/support system to provide continuity of care.

Work Experience:

3-5 years of Physical Therapy experience.

Position Qualifications:
  • Bachelors in physical therapy
  • Illinois PT license
  • CPR Certification
  • Experience in an outpatient setting preferred.
  • Bilingual English/Spanish a plus
  • Ability to communicate with staff at all organization levels
  • Effective written and verbal communication skills
