We are seeking a dedicated Physical Therapist to provide in-home therapy services to patients in the community. The Physical Therapist will evaluate patients, develop individualized treatment plans, provide therapeutic interventions, and monitor progress to help patients improve mobility, strength, balance, and overall function.
Responsibilities
- Conduct patient evaluations and assess mobility and physical limitations
- Develop and implement individualized treatment plans
- Provide physical therapy services in patients' homes
- Perform therapeutic exercises and mobility training
- Educate patients and caregivers on exercises and safe mobility techniques
- Document patient care and progress
- Communicate with other members of the healthcare team as needed
Requirements
- Current Physical Therapist license in the applicable state
- Graduate of an accredited Physical Therapy program
