What You Will Do
You will provide comprehensive physical therapy evaluations and treatments for PACE participants in a community-based setting. Your work will focus on fall prevention, mobility optimization, pain management, and functional maintenance. You will collaborate with an interdisciplinary team including physicians, nurses, social workers, and occupational therapists to develop holistic care plans. The pace of work is steady and sustainable, with an emphasis on quality over volume.

What You Bring
- DPT or Master's degree from an accredited PT program
- Current or eligible for Pennsylvania State PT license
- Interest in geriatric care and community-based health
- Strong assessment and communication skills
