How you'll make an impact in this role- Provide comprehensive rehabilitative care for a clinically diverse patient population, specializing in a wide cross section of neuro, ortho, and specialty areas like lymphedema and pelvic floor.
- Develop, implement, and regularly reassess individualized, patient-centered plans of care in collaboration with patients and families.
- Deliver evidence-based interventions across complex diagnoses, including neurological, oncological, orthopedic, cardiopulmonary, and medically complex conditions.
- Collaborate with the interdisciplinary team to coordinate care, optimize outcomes, and support safe, effective discharge planning.
- Schedule will have 2 evenings a week until approximately 7:15 p.m.
What minimum requirements you'll needLicensure / Certification / Registration:
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
- Physical Therapy (PT) credentialed from the Illinois Department of Financial and Professional Regulation prior to hire date or job transfer date required. A valid license pending letter will be accepted until license is issued.
Education:
- Bachelor's degree required. Master's OR Doctorate degree preferred.
What additional preferences we're seeking- 1-year minimum experience in lymphedema and/or pelvic health.
