Schedule: Monday-Friday, 8:00 AM - 5:00 PM (flexible based on patient visits)
Salary: $100,000 - $120,000 + $10,000 Sign-On Bonus

Job Overview

A growing home health organization is seeking Physical Therapists (PTs) to join its Skilled Home Health team serving patients throughout Hillsdale, Lenawee, and Monroe counties in Michigan.

In this role, the Physical Therapist will provide in-home therapy services focused on helping patients restore mobility, improve strength, and regain independence following illness, injury, or surgery. The PT will complete evaluations and start-of-care visits while collaborating closely with interdisciplinary care teams to develop individualized treatment plans that support optimal patient outcomes.

Compensation & Incentives
  • Base Salary: $100,000 - $120,000
  • $10,000 Sign-On Bonus
  • Productivity Bonus Structure
    • Expected productivity: 27 points per week
    • Additional compensation of $65-$75 per point for productivity above target
  • Annual bonus opportunities
  • Company vehicle available for therapists driving 1,400+ miles per month

Key Responsibilities
  • Provide physical therapy services to patients in their homes in accordance with physician orders and care plans
  • Perform patient evaluations and start-of-care visits
  • Develop and implement individualized therapy treatment plans
  • Educate patients and caregivers on exercises, mobility techniques, and safety strategies
  • Document patient progress and treatment outcomes in the EMR system
  • Collaborate with nurses, physicians, and interdisciplinary care teams
  • Complete required clinical documentation including OASIS assessments when applicable

Clinical Support & Workflow
  • PTs focus primarily on evaluations and start-of-care visits
  • Physical Therapist Assistants (PTAs) assist with follow-up visits
  • Dedicated scheduling team manages patient visit scheduling
  • EMR system: Homecare Homebase (HCHB)

Qualifications
  • Licensed Physical Therapist (PT)
  • Current Michigan PT license required
  • Home health experience preferred
  • OASIS documentation experience preferred
  • Experience with Homecare Homebase (HCHB) preferred
  • Strong clinical assessment and patient education skills
  • Ability to work independently while managing a patient caseload

Orientation & Training

New hires complete a two-day branch orientationfollowed by field precepting, with training length based on experience.

  • Current census of approximately 730 patients across the organization
