What You'll DoAs a Home Health Physical Therapist, you'll provide skilled physical therapy services under a physician-directed plan of care, helping patients safely recover from illness, injury, surgery, or hospitalization while maximizing their independence at home.
Your responsibilities will include:
- Performing comprehensive evaluations, reassessments, start-of-care visits, recertifications, and discharges
- Developing individualized treatment plans focused on each patient's functional goals
- Providing evidence-based interventions to improve strength, balance, mobility, endurance, and safety
- Educating patients and caregivers on home exercise programs, fall prevention, disease management, and safe mobility
- Collaborating closely with nurses, physicians, occupational therapists, speech therapists, and other members of the interdisciplinary care team
- Completing accurate, timely clinical documentation, including OASIS assessments
- Monitoring patient progress and modifying treatment plans to achieve the best possible outcomes
What We're Looking For- Graduate of an accredited Physical Therapy program (Bachelor's, Master's, or Doctorate)
- Current Connecticut Physical Therapist license
- Minimum of two years of clinical experience preferred
- Home health experience and OASIS documentation experience preferred, but not required
- Excellent clinical assessment, communication, and time management skills
- Ability to work independently while collaborating as part of a multidisciplinary team
- Reliable transportation and a valid driver's license

Compensation:The full time pay range for this position is $90,000 to $110,000 per year and represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary for this role will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications.
