RVNAhealth is seeking a compassionate and experienced Registered Nurse Case Manager to provide exceptional home health care throughout our Central Territory including towns of Weston, Wilton, and Redding, CT.

As an RN Case Manager, you'll manage your own patient caseload, coordinate care across an interdisciplinary team, and help patients recover, manage chronic conditions, and remain safely in the comfort of their homes. What You'll Do
  • Manage a caseload of home health patients, providing skilled nursing care and comprehensive case management.
  • Conduct Start of Care admissions, comprehensive assessments, and ongoing evaluations to identify patient and family needs.
  • Develop, implement, and update individualized plans of care in collaboration with physicians and the interdisciplinary care team.
  • Coordinate nursing, home health aide, therapy, and other support services to ensure high-quality, seamless patient care.
  • Monitor patient progress, communicate changes in condition, obtain physician orders as needed, and adjust care plans accordingly.
  • Educate patients and caregivers on diagnoses, medications, treatments, and self-management strategies to promote independence.
  • Complete timely, accurate clinical documentation in accordance with agency, state, and Medicare guidelines.
  • Participate in discharge planning and ensure smooth transitions when care goals have been achieved.
What You'll Bring

Compensation:

Our target pay range for this position is $97,000 to $105,000 per year. This pay range represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary for this role will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications.
