Saratoga Medical is seeking a qualified
Pharmacist to support healthcare services in
Maricopa County, Arizona. The ideal candidate will possess a current Arizona pharmacist license and meet all credentialing requirements for the assignment.
Required Qualifications:- Doctor of Pharmacy (PharmD) degree from an accredited program
- Current, valid, unrestricted Pharmacist license issued by the Arizona State Board of Pharmacy
- Current Primary Source Verification (PSV) from the Arizona State Board of Pharmacy
- Minimum of one (1) year of experience as a Pharmacist
- Ability to meet all credentialing and compliance requirements
Required Documents:- Copy of PharmD diploma and/or transcripts from an accredited program
- Copy of current Arizona Pharmacist license
- Copy of current PSV from the Arizona State Board of Pharmacy
- Documentation supporting at least one (1) year of pharmacist experience
