Job Summary and QualificationsMaintains current knowledge of therapeutics necessary to be involved in all aspects of patient care decisions.
Interprets drug information requests, utilizes available resources and provides a response appropriate to the understanding of the end user.
Provides in-service education within and outside the department.
Communicates effectively (verbal and written) with staff and other customers.
Maintains an ongoing drug use evaluation program.
Documents and participates in an adverse reaction (ADR) monitoring program.
Assists the Director of Pharmacy in the development of policies and procedures.
Reviews and enters medication orders.
Coordinates and reviews dispensing of all medications.
Assesses all medication orders for appropriateness.
Supervises preparation and dispensing of sterile products including TPN and chemotherapy.
Assures controlled substance dispensing and documentation throughout LewisGale Hospital Montgomery.
Communicates effectively with pharmacy staff, patients and all members of the healthcare team.
Performs responsibly as supervisor when necessary.
Participates in Performance Improvement/Patient Safety Processes.
Other duties as assigned.
What qualifications you will need:EXPERIENCE
Required: Prior experience as a pharmacist. New graduates must have gained pharmacy experience as an Intern/extern.
Preferred: Prior experience in hospital pharmacy with clinical emphasis.
EDUCATION
Required: Bachelor of Science in Pharmacy or Doctorate of Pharmacy from an accredited school of pharmacy; License in good standing as a Pharmacist in the Commonwealth of Virginia
Preferred: Advanced degree/certification in Pharmaceutical Care.
