Job Description:
Purpose of Job:
Improve the overall health of the communities we serve by providing pharmaceutical care in the following ways:
Essential Duties and Responsibilities:
Reviews prescriptions and prepares medications for dispensing.
Counsels9 patients on medication drug interactions, side effects, dosage, and storage of pharmaceuticals.
Reviews and authorizes prescriptions with delegated prescriptive authority under state approved Collaborative Drug Therapy Agreements.
Aids in helping patients obtain insurance/patient assistance options.
Acts as a part of the care team providing consultation for specific disease states.
Maintains established procedures of quality assurance, security of controlled substances, and HIPAA.
Directs pharmacy technicians, assistants, and interns.
Accesses computer for data entry, pharmacy files, patient charts, and inventory.
Utilizes innovative technology with electronic health records and Parata.
Performs other duties assigned, including supporting the CHAS Health Mission and Core Values.
Qualifications:
Education/Experience: Rph or PharmD from accredited school of pharmacy. Current valid licensure in the state of Washington and/or Idaho required. Prior non-profit experience preferred.
Skills: Excellent oral and written communications skills required. Computer skills required. BLS (CPR/AED)
required. Commitment to supporting a safe and respectful environment is required.
Physical Demands:
Pharmacists must be able to move around the facility between 1/3rd and 2/3rds of the day, while sitting at a workstation occurs under 1/3rd of the day. Using hands and reaching with arms occurs over 2/3rds of the day. Communicating is also required over 2/3rds of the day, while climbing, stooping or crawling is minimal. Lifting up to 10 lbs occurs occasionally, while 25-50 lbs occurs infrequently.
