Reports to: Manager
Salary Range: $45.85 to $57.31 per hour
Hours: 37.5 hours per week
Shifts: Monday to Friday
Status: Permanent Full-Time

Posting Summary

The inpatient/ Outpatient Patient Care Coordinator will support:

The Classical Hematology Program which includes the Thrombosis Clinic, Red Blood Cell Disorders Clinic, and General Hematology Clinic. The Palliative Care Program which spans both inpatient and outpatient services.

Together, these programs provide specialized, interdisciplinary care for patients with complex non-malignant hematologic conditions, cancer-related pain needs, and end-of-life care needs across oncology diagnoses.

The portfolio includes leadership responsibilities for administrative staff, registered nurses, clinical coordinators, and allied health professionals. These teams work collaboratively within an interdisciplinary model alongside nurse practitioners, physicians, physician assistants, and clinical nurse specialists to support high-quality, coordinated care across inpatient, outpatient, and specialty clinic settings.

Duties

The PCC has clinical and leadership expertise. The principal responsibilities within the site(s) include:
  • Ensuring efficient day to day operations of clinical areas
  • Ongoing continuous improvement and project management practices to strengthen clinic processes
  • Performing financial/administrative responsibilities to ensure efficient and effective site operation
  • Contributing to staff development by promoting and ensuring a supportive/collaborative learning environment within the service(s)/cluster area based on the principles of adult learning and practice standards for nurses, students and ancillary staff
  • Performing human resources responsibilities in collaboration with the nurse manager
  • Liaising with members of the multi-disciplinary team to ensure high standards of quality and optimal management of patient care outcomes (including data collection/reporting)


Qualifications
  • At minimum, a completion of BScN Baccalaureate in Nursing or Baccalaureate level education with Diploma in Nursing required
  • Completion of a Master's degree in Nursing preferred
  • Basic Cardiac life Support (B.C.L.S.) program
  • Three (3) years nursing experience, including experience as required by area of responsibility and experience working in a supervisory capacity
  • Current certificate of competency (licensure) from the College of Nurses of Ontario required
  • Demonstrates membership within a professional nursing association and/or nursing interest groups related to practice
  • Demonstrated strong leadership and motivational skills
  • Experience with project management and/or Lean process improvement an asset
  • Demonstrates ability to work in a team environment and collaborates with others in assisting with the delivery of care to patients
  • Demonstrated leadership abilities
  • Excellent skills in recognizing, analyzing and resolving problems
  • Excellent time management skills
  • Excellent decision making and problem-solving skills
  • Clinical and patient teaching skills
