Job Description:

Participates in interdisciplinary team meetings to discuss patient care and progress. Provides emotional support and stress management techniques to patients and families. Maintains accurate and timely documentation of all patient interactions and interventions. Engages in continuous professional development and stays current with best practices in clinical social work. Conducts comprehensive psycho-social assessments to identify patient needs and barriers to healthcare goals. Provides individual and family counseling to help patients cope with medical issues and life changes. Educates patients and families about available community resources and assists with referrals. Supports patients with end-of-life planning, including advance directives and palliative care options. Addresses issues of abuse, neglect, and domestic violence, providing appropriate interventions and referrals. Other duties as assigned. Collaborates with healthcare team members, including physicians, nurses, and case managers, to develop and implement care plans. Facilitates discharge planning by coordinating post-acute care and community resources. Offers crisis intervention services for patients and families experiencing acute distress.Knowledge, Skills, and Abilities:
