DescriptionHospice On-Call Registered Nurse (RN) - After Hours | Bringing Comfort When Families Need It Most Wellspring Lutheran Services - Frankenmuth Hospice | Serving Saginaw, Bay City, Frankenmuth, Caro and surrounding areas | Full-Time, Hybrid Remote | 7 Days On, 7 Days Off | Salary $85,000 As our Hospice After Hours RN, you'll serve as the primary clinical resource for patients and families during evenings, nights, weekends, and holidays. You'll provide skilled nursing care, symptom management, telephone triage, admissions, crisis intervention, and end-of-life support while helping ensure patients receive the comfort-focused care they deserve whenever a need arises.
Most importantly, you'll help families feel supported, cared for, and never alone during some of life's most meaningful and vulnerable moments.
What Your Impact Will Look Like You'll be the reassuring voice on the other end of the phone when a caregiver has concerns in the middle of the night and isn't sure what to do next.
You'll use your clinical judgment to assess symptoms, prioritize patient needs, and determine when intervention is needed, helping families feel confident and supported during uncertain situations.
You'll provide hands-on nursing care wherever patients call home, whether that's a private residence, assisted living community, skilled nursing facility, or hospital setting. Your presence will help ensure patients remain comfortable, safe, and cared for regardless of the hour.
You'll help manage pain, respiratory distress, anxiety, agitation, nausea, and other end-of-life symptoms, allowing patients to spend more meaningful time focusing on what matters most rather than on discomfort.
You'll guide families through difficult conversations and provide education about medications, symptom changes, disease progression, and the hospice journey. During emotional moments, you'll become a trusted source of support and understanding.
You'll help welcome patients into hospice services through after-hours admissions, giving families immediate access to comfort-focused care when they need it most.
There will be nights when you're helping manage an urgent symptom crisis. Other times, you'll be offering reassurance to a worried spouse, admitting a new patient into hospice care, or supporting a family through the final hours of a loved one's life.

What You'll Bring- Current Registered Nurse (RN) license through the State of Michigan.
- Graduate of an accredited Registered Nursing program.
- Minimum of one year of nursing experience required.
- Hospice, palliative care, oncology, home health, ICU, ER, or geriatric nursing experience preferred.
- Strong assessment, critical thinking, and clinical decision-making skills.
- Thorough understanding of hospice philosophy and end-of-life care.
- Ability to work independently and manage multiple priorities.
- Excellent communication and interpersonal skills.
- Ability to provide emotional support to patients and families during difficult situations.
- Valid driver's license, reliable transportation, acceptable driving record, and proof of automobile insurance.

Schedule- Full-time
- 7 Days On / 7 Days Off rotation
- Primary after-hours coverage for on-call, including scheduled evenings (admits / visits as needed), overnight hours, every other weekend and 2 holidays per year.
- Respond to patient needs throughout the assigned service area
- Combination of telephone triage, on-call support, patient visits, admissions, and end-of-life care responsibilities during scheduled coverage periods.
