Minimum Qualifications:- Must be licensed as an Occupational Therapist in the state of Oregon
- Graduate from an accredited program with a degree in Occupational Therapy
- CPR Certification
- National Provider Identifier (NPI) required upon hire
Preferred Qualifications:- Experience in Home Health
- Organizational skills to facilitate program development
Responsibilities:- Establish treatment plan appropriate to physician orders
- Prepare/complete clinical and progress notes
- Teach patient and family/caregivers in meeting individual treatment goals
