Qualifications

• Active Texas COTA license in good standing
• Demonstrated clinical judgment and ability to work independently
• Excellent communication and documentation skills
• Valid driver's license with willingness for daily travel
• Prior home health experience (preferred)
• Familiarity with electronic medical records (EMR) systems (preferred)

Responsibilities

• Provide skilled COTA services in patient homes under an OT's plan
• Implement treatment plans focusing on activities of daily living (ADLs) and safety
• Monitor and report patient progress and concerns to the supervising OT
• Collaborate with nursing and care teams to address patient needs
• Maintain accurate and timely documentation of patient care
