Behavioral Health Network (BHN) is hiring a
Nurse Supervisor for our
One Care Program.

As the
Nurse Supervisor, you'll provide clinical and administrative leadership to our Nurse Care Managers and MDS Assessment Nurses, ensuring high-quality, person-centered care coordination that helps individuals navigate the healthcare system, remain engaged in community-based services, and reduce avoidable emergency department visits and hospitalizations.

What You Will DoAs the Nurse Supervisor, you will provide clinical and administrative leadership to Nurse Care Managers and MDS Assessment Nurses while ensuring high-quality, person-centered care coordination for individuals enrolled in the One Care program.
You will:
- Provide direct supervision, coaching, mentorship, and performance management to Nurse Care Managers and MDS Assessment Nurses.
- Ensure appropriate nursing staffing, workload distribution, and clinical coverage.
- Conduct regular supervision meetings, case consultations, performance evaluations, and support staff professional development.
- Monitor productivity, documentation quality, assessment timeliness, and compliance with program and regulatory requirements.
- Partner with program leadership to address performance concerns and implement improvement plans when needed.
- Support onboarding, orientation, and ongoing training for nursing staff.
- Serve as a clinical resource for complex medical, behavioral health, substance use, and social care needs.
- Oversee the completion, quality, and timeliness of MDS assessments in accordance with Commonwealth Care Alliance (CCA) and MassHealth requirements.
- Complete MDS assessments in home and community settings as needed.
- Ensure timely completion of post-discharge assessments, medication reconciliations, and transitions of care.
- Promote integrated care planning that addresses physical health, behavioral health, long-term services and supports (LTSS), and social determinants of health.
- Review care plans, documentation, and quality metrics to support continuous quality improvement and regulatory compliance.
- Utilize program data and performance metrics to improve clinical outcomes, reduce preventable emergency department visits and hospitalizations, and strengthen care management practices.
- Collaborate with interdisciplinary teams, providers, CCA, and community partners to coordinate comprehensive care.
- Support implementation of new workflows, policies, and operational initiatives.
- Facilitate nursing meetings, case reviews, and interdisciplinary collaboration.
- Maintain a limited direct caseload and provide direct clinical support during staffing shortages or high-volume periods.
- Model BHN's commitment to integrated, person-centered, trauma-informed care.
Who You AreYou are a compassionate and collaborative nursing leader who is passionate about supporting both staff and individuals served while advancing integrated healthcare.
You have:
- Active Registered Nurse (RN) license in Massachusetts.
- Minimum of two (2) years of clinical experience in care management, case management, integrated care, community health, or a related setting preferred.
- Previous nursing leadership or supervisory experience preferred.
- Experience working with individuals with complex medical, behavioral health, substance use, and social needs.
- Knowledge of One Care, MassHealth, Commonwealth Care Alliance (CCA), and care management models preferred.
- Familiarity with MDS assessments, care planning, and regulatory requirements preferred.
- Strong understanding of interdisciplinary care coordination and social determinants of health.
- Experience with quality improvement, documentation review, and performance management.
- Strong leadership, communication, organizational, and problem-solving skills.
- Proficiency with electronic health records (EHRs), Microsoft Office, and clinical documentation systems.
- Bilingual Spanish/English preferred.
- Valid driver's license and reliable transportation.
Salary$80,000-$98,000 annually, based on experience.
