Duties & Responsibilities

Position Overview:

Reporting to the Chief Medical Officer and working in collaboration with the attending physicians, the nurse practitioner is responsible for providing medical management for geriatric and non-geriatric nursing facility residents including primary care, management of acute illness, and transition of care.

Essential Functions:
  • Perform services and medical management of nursing facility residents in adherence with CMS and NYS DOH requirements for the provision of medical services.
  • Provide medical management for geriatric and non-geriatric nursing facility residents including primary care, management of acute illness, and transition of care.
  • Coordinate behavioral health services for individuals with maladaptive behaviors.
  • Collaborate with attending physician with assigned patient case load.
  • Perform medical assessment for new admissions and readmissions and write orders for required medications, treatments, diagnostic studies and other necessary care and treatment.
  • Perform required medical visits based on resident need, facility policies and regulatory requirements.
  • Write timely orders and progress notes.
  • Initiate referrals for consultant and specialty services as required and write orders for services needed.
  • Write orders for rehabilitation services.
  • Participate in advance directives planning with the resident and representatives and write related orders.
  • Participate in discharge planning process and write related orders.
  • Facilitate the emergency discharge of residents to acute hospitals as well as planned discharges for medical treatment
  • Provide medical leadership for team-coordinated Quality Assurance Performance Improvement initiatives


Minimum Qualifications
1. A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and
2. Current certificate as a Nurse Practitioner issued by the NYSED; and
3. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).

Department Preferences

  • Long term Care and/or Geriatric experience.
  • Board Certified
  • Experience with Post-Acute EMR
  • Familiarity of CMS and NYS DOH regulations regarding medical management of nursing facility residents


Additional Salary Compensation

In addition to base compensation, you may also receive the following based on the Collectively-Bargained Agreement:

An annual education differential if you currently possess an appropriate degree in nursing or an allied health discipline from an accredited college or university, as listed below:

Bachelors of Science in Nursing (BSN) or Bachelors in allied health discipline = $900
Masters of Science in Nursing (MSN) or Masters in allied health discipline = $1,200
Doctorate in Education, Nursing Practice or PhD in Nursing (DNP) = $2,500

An annual certification differential of $1,878 if you currently hold an appropriate certification that is nationally recognized in the discipline or area of specialization for which you are employed.

An annual nursing experience differential ranging from $1,097 up to $9,100 for up to 10 years of experience if you have outside nursing experience as a Nurse Practitioner.
