Qualifications

• Valid nurse practitioner or physician assistant license required in the state of operations
• Current board certification in relevant field necessary
• Minimum Bachelor’s and Master’s Degree in Nursing from accredited institutions
• Experience in nursing is required; prior Nurse Practitioner experience preferred but not mandatory
• Strong critical thinking skills essential, especially in dealing with elderly and chronically ill patients
• Leadership, interpersonal, and problem-solving skills are advantageous
• Excellent written and verbal communication skills needed
• Strong organizational skills and punctuality are critical

Responsibilities

• Provide direct patient care services in a Post-Acute Care setting
• Assess and manage patient care plans based on individual needs
