Graduated from an accredited School of Nursing and Nurse Practitioner Program.
Master's degree with at least one year of Nurse Practitioner experience.
Minimum of one year in hospice or palliative care.
Current DEA license required.
Active driver's license and reliable transportation necessary.
Responsibilities
Provide medical care and palliative management for terminal illnesses.
Conduct face-to-face hospice visits to assess patient eligibility.
Advise the hospice medical director on patient certification and recertification.
Collaborate with the interdisciplinary team to develop patient care plans.
Stay updated on Medicare/Medicaid regulations and hospice service compliance.

Our Hospice Nurse Practitioner Will:
Provide medical care, palliate, and manage terminal illness in compliance with the state Nurse Practice Act under the supervision of collaborating physician.
Complete Hospice fact-to-face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based on the hospice conditions of participation and local coverage determinations.
Make recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care.
Communicate and collaborate with the interdisciplinary group to create, review, and revise the patient's plan of care.
Maintain knowledge of and compliance with current Medicare/Medicaid, state/federal rules, and regulations for hospice services.
Fulfill the obligation of requested and/or accepted assignments.
Requirements
Our Hospice Nurse Practitioner Has:
Graduated from an accredited School of Nursing and accredited Nurse Practitioner Program.
A master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience.
A current DEA license.
