Job Description
Key Responsibilities:
•
Provides comprehensive primary care to residents.
•
Is knowledgeable of the Resident Bill of Rights and endeavors to respect and promote it.
•
Conducts person-centered health assessment/physical of residents on admission with specified time frame.
•
Comply with applicable provincial legislation pertaining to long-term care, Fixing Long- Term Care Homes Act
•
Participate in regular focused resident and family care conferences.
•
Manages the care of residents by providing pharmacological, complementary and/or counseling interventions.
•
Provides outreach and transitional services to LTCH residents who return to the community.
•
Collaborates and consults with physicians, director of nursing and personal care nursing staff, interdisciplinary team and external resources regarding resident plan of care.
•
Provides leadership and involves the interdisciplinary team in quality improvement initiatives.
•
Provides formal and informal teaching and coaching in the management of clinical care.
•
Adheres to responsibilities and requirements for reporting adverse resident occurrences subject to regulatory and legislative direction.
