We are seeking a Family Nurse Practitioner for our Long Island City Health Center located at 36-11 21st Street, Long Island City, NY [redacted].
Reporting to the Medical Director, this individual will be responsible for providing evidence-based Primary Care to our patients. This individual is an integral of the collaborative care team which includes nursing staff and other services such as Health Education, Nutrition, Social Work and Behavioral Health.
The ideal candidate will have a valid Family Nurse Practitioner N.Y.S. License to deliver medical care and at least two (2) year of post-graduate/residency experiencedelivering care in a Primary Care/Ambulatory Care setting.
